Support Coordination

What is Support Coordination?

Support Coordination is an NDIS funded service that’s designed to help NDIS participants (and, if required, their family or representatives) to coordinate the support services in their NDIS Plan. 

Am I eligible for Support Coordination?

If you’re living with disability, your plan from the National Disability Insurance Scheme (NDIS) gives you a set allocation of funding for a range of different support services.

The support funding in your plan is very specific and relates to helping you to achieve your personal goals. This might include a range of services such as in-home support, social and community access or therapy sessions. Your NDIS plan will state if you are eligible for Support Coordination.

Why do I need Support Coordination?

Support Coordination is designed to work with NDIS Participants to build and maintain a support network. This involves:

  • supporting you to understand your NDIS plan.
  • assisting you to have choice and control over your NDIS services and other supports. 
  • assist to get you through any challenges that might come up with your support services; and 
  • capacity building.

What is the difference between Support Coordination and a Plan Manager?

A Support Coordinator’s role is to help NDIS participants to understand their NDIS Plan, identify their needs and goals, monitor their progress and connect with the people and support that will enable them to live with choice and control.

 

A Plan Manager is responsible for the financial/administrative side of a participant’s NDIS Plan. For example, the Plan Manager is responsible for organising payments, processing claims and invoices and tracking budgets. If you require plan management services, your support coordinator can help you to arrange this.

How can the Support Coordinator help with an NDIS plan review?

A support coordinator can help participants and their representatives before, during and after an NDIS Plan Review meeting.

Before

By producing a Plan Review Report to outline the services that have been arranged to meet the NDIS participant’s goals. This report includes all assessment reports (to demonstrate the progress towards achieving goals while using a service, during the Plan period) and includes recommendations for future services.

By researching specialized services and equipment, as required.

During

By supporting participants and their family or representatives on the day of the Plan Review meeting and helping them to navigate the process.

By acting as a point of contact for National Disability Insurance Agency (NDIA) personnel – to provide them with the clarification and additional information required to achieve a high-quality NDIS Plan for the participant.

Can Your Way Disability SA provide Support Coordination and other NDIS supports?

Yes. Your Way Disability SA has strategies in place to prevent any conflict of interest.

Psychosocial Recovery Coaching

What can a psychosocial recovery coach help with?

  • Implement a recovery plan to help you become more independent
  • Help you get the most from your NDIS Plan
  • Assist with coordination of NDIS supports
  • Provide coaching to increase skills and capacity such as motivation, strengths, resilience, and decision-making
  • Connect you with mainstream support.

Who will be funded for Psychosocial Recovery Coaching?

Participants with a primary diagnosis of psychosocial disability can request for this support to be put in their plan if it meets the reasonable and necessary criteria. Recovery coaching is not currently available to participants who have psychosocial disability as their secondary diagnosis.

How many hours will people be funded for Psychosocial Coaching?

We have started to see Psychosocial Recovery Coaching include people’s plans with around 90 to 100 hours, which is more hours than we tend to see for support coordination. But the NDIA has been clear that there are no minimum or maximum hours for recovery coaching and that will be based on an individual’s need.
