Support Coordination & Psychosocial Recovery Coach

Support Coordination is a service we offer under the National Disability Insurance Scheme (NDIS). It assists participants in navigating the NDIS system, implementing their plans, and coordinating supports from various service providers. Support coordinators work with participants to identify their goals and needs, develop a plan to achieve those goals, and connect them with the appropriate supports and services.

A Psychosocial Recovery Coach, on the other hand, is a professional who provides support and guidance to individuals with mental health challenges. They focus on helping individuals develop skills and strategies to manage their mental health, improve their quality of life, and work towards their recovery goals. Psychosocial Recovery Coaches often work with individuals in areas such as building social connections, increasing independence, and improving overall well-being.

In summary, Support Coordination is focused on assisting individuals with disabilities in accessing and coordinating supports under the NDIS, while a Psychosocial Recovery Coach provides support to individuals with mental health challenges to improve their mental health and well-being.

Support Coordination is an NDIS funded service that’s designed to help NDIS participants (and, if required, their family or representatives) to coordinate the support services in their NDIS Plan.

Psychosocial Recovery Coaches implement a recovery plan to help you become more independent and help you get the most from your NDIS Plan. We will assist with coordination of NDIS supports, provide coaching to increase skills and capacity such as motivation, strengths, resilience, and decision-making by connecting you with mainstream support.

Community Support & Respite

Community support and respite services are designed to provide assistance and relief to individuals with disabilities, chronic illnesses, or other challenging circumstances, as well as their families and caregivers.

Community support services typically involve a range of supports that help individuals participate in their communities and maintain their independence. This can include assistance with daily living activities, such as personal care, household tasks, and transportation, as well as support to access community resources, engage in social activities, and develop life skills.

Respite services, on the other hand, offer temporary relief for caregivers by providing care and supervision to individuals with disabilities or special needs. Respite services can be provided in various settings, such as in the individual’s home, in a community setting, or in a residential facility. Respite care can range from a few hours to several days, depending on the needs of the individual and their family.

Overall, community support and respite services aim to enhance the quality of life for individuals with disabilities and their families by providing practical assistance, promoting social inclusion, and reducing caregiver stress.

Positive Behaviour Support & Developmental Education

Positive Behaviour Support (PBS) is an evidence-based approach to understanding and managing challenging behaviours. It focuses on proactive strategies to teach and reinforce positive behaviours, rather than using punitive measures to address challenging behaviours. PBS is based on the belief that all behaviour serves a purpose, and by understanding the function of behaviour, individuals can develop more effective ways to meet their needs.

PBS involves a collaborative and person-centred approach, where the individual, their family, and support team work together to identify triggers for challenging behaviours, develop strategies to prevent these behaviours, and teach alternative, more appropriate behaviours. The goal of PBS is to enhance the quality of life for individuals by promoting meaningful social interactions, skill development, and independence.

Our Developmental Educators are an allied health professional who provides specialised support and intervention to individuals with developmental disabilities. These educators work with individuals to develop skills and strategies to enhance their independence and quality of life.

Some examples are:

Conducting assessments to identify the individual’s strengths and areas for development.

Developing personalised goals and strategies to address the individual’s specific needs.

Providing support and guidance to the individual and their support network, including family members, carers, and other professionals.

Implementing interventions and programs to help the individual achieve their goals.

Monitoring progress and adjusting strategies as needed.

The Developmental Educators play a crucial role in supporting individuals with developmental disabilities to reach their full potential and improve their overall quality of life.
