 Support Coordination is an NDIS funded service that’s designed to help NDIS participants (and, if required, their family or representatives) to coordinate the support services in their NDIS Plan.

Support Coordination

This support will assist you to build the skills you need to understand, implement and use your plan. The Support Coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community. Capacity Building support. We can complete research; make enquiries on the participants behalf; negotiate service agreements; and ensure your support is organised inline with NDIS budgets. When it comes to engaging service providers, we support participants to speak to services to negotiate days/times that suit. 

Psychosocial Recovery Coach

The aim is to increase participants’ social and economic independence and participation through the provision of capacity building support. Psychosocial Recovery Coaches will focus on coaching participants to have greater participation in managing their lives and will collaborate with other services to support and underpin these outcomes. Psychosocial Recovery Coaches will provide support to people with psychosocial disability to increase their independence, social participation, and economic participation. People will be assisted to take more control of their lives and to better manage complex challenges of day to day living. Through recovery-enabling relationships and skilled coaching, people will be supported to build capacity, including strengths and resilience. Psychosocial Recovery Coaches will work collaboratively with people, their families, carers, and other services to design, plan, implement and review a recovery plan.

Positive Behaviour Support

Positive Behaviour Support (PBS) primary focus is to support you to live a meaningful and fulfilled life. Focusing on person centred intervention, safeguarding, quality of life, reasons for behaviour, capacity building and the environment around the person. We
provide interventions such as strategy planning, environmental adjustment, analysis of behaviour and staff training and development opportunities. We work creatively to assist,
support, and build therapeutic relationships with our clients.
Your Way Disability SA PBS practitioners are registered practitioners, who are considered suitably qualified by the NDIS commission, and are required to adhere to legislation under the NDIS framework to promote best practice. To access this service, “Improved Relationships” funding will need to be written into your
NDIS plan.
